• SCRIP Enrollment Form
  Return this completed form to Tina Kurtz at The Visitation Academy.
  The enrollment form must be on file before purchases can be made.
   

• SCRIP Printable Order Form
  Due to changing percentage rebates and special offers, this form will be updated regularly so you will always have the most accurate information.
  Return the completed order form with your check or credit card number to Visitation Academy by Tuesday of every week during school.

  Click on the "shop with scrip" icon on the next page to create a personal account and place your order online with any of the providers listed. Send in your payment check to the school or contact Tina Kurtz to register a credit card number to be kept on file.
